On the mainland, a supervising physician who isn’t at one location can reach another. In Hawaii there is no backup within driving distance, because there is no driving. A center on Maui, Hawaii Island, or Kauai depends entirely on the physicians who live on that island, and when one is unavailable there is no regional pool to draw from — only a flight.
That reshapes the economics. Oahu concentrates most of the state’s specialist capacity, which leaves neighbor-island facilities recruiting against a market where housing costs are high, the candidate pool is small, and a single departure can leave a service line without coverage for months.
